Refunds/Returns
All sales are final. Each unit is custom made to fit each individual client. Please make sure your measurements and all other unit choices are correct BEFORE you send in your payment. NO CHANGES will be made to ANY order after it is sent in. I do quality control on each and every unit that is created.
The only time a refund or exchange will be made is if the unit arrives damaged, in this case I need the unit to be sent overnight mail to: Pure Glamour 6733 Castor Avenue, Phila., PA 19149. The unit must be intact and no lace trimmed At ALL. I will need a letter emailed to me describing exactly what Measuring for your cap sizeong with the unit.
Use the following chart as a guide line for measurements. Print the page and write down your measurements. Check them twice for accuracy. When you place any order with Pure Glamour Salon, your measurements will be required
Step 1. Flatten your hair, use a wig cap or braid your hair close the head to make your measurements. Step 2. Use a cloth/Vinyl/plastic tape measure Step3. Use the chart below to take your measurements. Please sure to do this at least twice. Step4. Record your measurements
Important to note: Do not pull the tape measure when measuring. Pulling the tape measure will give you inaccurate measurements.
Guide to Filing an Insurance Claim
1 .Most nsurance policies cover “Prescribed Cranial Prosthesis” though does not cover wigs.
2. The Insurance C.P.T code most commonly used is CPT-ORN-1.
3. Get a letter and/or prescription from your doctor.
4. Make sure that the letter or prescription is for “Prescribed Cranial Prosthesis”
5. It should not say wig-if it does, have it redone, otherwise your claim will most likely be refused.
6. Have a receipt from the person performing the service (from whom you purchase the Cranial Prosthesis). Make sure it says “Prescribed Cranial Prosthesis.”
7. Have the person who performed the service signoff on your claim, i.e. the “Dr.s” portion of your claim form.
Waiting for your claim to be processed – prepare yourself for having claim denied.
A. Know your rights as the insured party.
B. Obtain a copy of your FULL contract with your carrier (most of us only have a “Summary of Benefits” provided by the employer).
C. Read and know your contract.
1. Pay attention to the “exclusions” Wigs are excluded for cosmetics reasons.
2. Pay special attention to Prosthesis coverage. Most policies do not spell out exclusions under Prosthesis, and this is how you are likely to have your claim accepted.
Submitting your appeal – (after denial of claim as not a covered benefit)
A. Write asking for a review by the Medical Review Board.
B. Attach copy of original claims.
C. In your letter, inform them of the difference between a wig and Prosthesis. (A wig is a fashion item and not a necessity. Prosthesis is a necessity comparable to plastic surgery for a burn victim).
D. Ask for a written reply. (Companies try to avoid putting anything in writing).
E. Address your letter to a supervisor in the Claim Department.
